Mahesh, 98 kgs Diabetic, Hypertensive, Obese was treated successfully

Mahesh (name changed) was a 58 year old male with history of Diabetes Mellitus and Hypertension since 12 years requiring 150 units of Insulin and five oral hypoglycaemic agents per day for the control of his blood sugar levels. He had developed diabetic neuropathy affecting his legs and arms. Neuropathy which causes paresthesia and numbness was hampering his routine activities. His diabetes and neuropathy could not be controlled in spite of high doses of insulin, oral hypoglycaemic agents and nerve regeneration medications. Fed up with the disease progression he opted for “Metabolic surgery” for the remission of his diabetes and diabetic neuropathy . On evaluation, his Body Mass Index (BMI ) was 34.7 ( Weight 98 kgs, Height 168 cms). His HBAIc levels was 9.5% suggesting poor glycemic control. He was also deficient in Viatmin D3 levels. Endoscopy showed features of Diabetic Gastropathy suggesting neuropathic involvement of gastrointestinal tract. Urine examination showed features of microalbuminuria suggesting his diabetes has started affecting his kidneys also. After complete preoperative evaluation, he underwent Laparoscopic Roux – en – Y Gastric Bypass (RYGB ) under general anaesthesia on 10th July 2012. To his surprise, his sugar levels were normal the day after surgery without any Insulin or oral hypoglycaemic agents. Diabetes was controlled before he was discharged from the hospital, on the 3rd day following surgery. On first month of follow up, he had lost 12 kgs with complete resolution of his diabetes requiring no medications for the control of his blood sugar levels. Urine microalbuminuria also disappeared indicating reversal of early nephropathy changes. On follow up after five months he had come down to 75 kgs with a new lease of life. He now does not require any treatment for his diabetes, hypertension or neuropathy.
